[MKDoc-modules] MKDoc::Text::Structured usability issues

Couple of things:

headlines + paragraphs
==================

Writing:

    headline
    -----------
    paragraph


Instead of

    headline
    -----------

    paragraph

(presumably this is a bug)


pre-formatted text
==============

Using just indentation might not be such a good idea, since often people 
start paragraphs with whitespace, i.e.

    this is a paragraph
blah blah patati patala
blih blah.

One solution to this would be to require a carriage return before and 
after all pre-formatted text.

So presumably, this is a bug too :-)
